Player Questionnaire - Step 1

Your tryout registration includes four required steps: complete the Player Questionnaire, sign the Liability Release, sign the Photo Release, and submit the $40 Tryout Fee.

Important: Your registration is not complete until all four steps—including payment—have been submitted. Please complete each step before arriving at tryouts.
